"Take Control of Your Health"

"Take Control of Your Health" is a chronic disease self management program that was developed by Standard University. Examples of chronic conditions are arthritis, diabetes, high blood pressure, Alzheimer's disease, cancer, chronic obstructive pulmonary disease, asthma, depression, mental illness and addiction. This program has been used for adults ages 18+ to give the participants skills to manage one's own health. Results have shown an increase in physical activity, improved nutrition, decreased stress, improved communication skills, and improved planning and goal setting behaviors. This is free peer leader certification training that lasts 4 days, (MUST attend all 4 days). You will learn to facilitate a program that helps people better manage chronic symptoms, work with health care professionals, set weekly goals, problem solve, relax, handle difficult emotions, eat well, and exercise safely and easily.
